本片的配乐在当年获得奥斯卡最佳电影配乐的大奖。影片使用的背景音乐非常丰富,有军乐、俄国民谣、爵士、华尔滋……,其中最为大家所熟悉的旋律莫过于后被填上歌词的“Somewhere, My Love”,在片中曲名是《拉娜之歌》( Lara s Theme),专为日瓦戈医生的爱人拉娜所作,旋律优美舒缓,漫溢对生活的热情。配乐的作曲与指挥是法国人莫里斯·贾尔( Maurice Jarre),后来,他还为大卫·里恩的电影《雷恩的女儿》的担任配乐,1990年的《第六感生死恋》的配乐也出自他的手笔。
